About the Role

The Intensive Family Coordinator will provide professional case management and direct advocacy services to a caseload of families, including completing family assessments and creating individualized service plans.

Responsibilities

  • Completing family assessments
  • Creating individualized service plans
  • Reporting to the Director of the Program
  • Recruiting foster parents
  • Completing home studies for the referring authority
  • Locating family resources
  • Providing case management services including intakes, safety planning, team meetings, ISP development, 24/7 support, case contacts, and case record documentation
  • Traveling as necessary
  • Working nights and weekends
  • Providing 24/7 crisis support to the caseload

Requirements

  • Minimum High School Diploma or GED Equivalent
  • Experience working with youth
  • Strong organizational and time management skills
  • Proficiency with Microsoft Office
  • Reliable transportation, current driver's license, and current auto insurance coverage
  • Flexible hours; possibility for evenings and weekends

Salary/Compensations
