About the Role
Heartline Staffing is recruiting a Primary Care Physician to provide value-based healthcare to Medicare patient populations. Physicians practice at a single medical center supported by large care teams, allowing providers to focus on quality of care rather than volume of services. The value-based care model is structured around health outcomes, preventive medicine, low hospital admissions, and patient experience.
Responsibilities
- Assess and diagnose patients
- Oversee, direct, and administer primary care
- Prescribe and administer pharmaceutical treatments and medication
- Maintain Patient Electronic Medical Record data using Canopy and Greenway
- Collaborate with regional and central leadership to meet health quality goals
- Work with Practice Managers to direct and manage the center care team
- Perform other duties as assigned
Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate REQUIRED
- Internal Medicine or Family Medicine Board Certified or Board Eligible REQUIRED
- Active, non-probationary, unrestricted state license REQUIRED
- Active DEA license REQUIRED
- U.S. work authorization REQUIRED
- Fellowship training in Geriatrics
- Experience practicing medicine among geriatric populations
- Experience operating in a Value-Based Healthcare Model
- Experience working in a collaborative setting to ensure positive health outcomes
- Experience in outpatient primary care settings
- Bilingual proficiency where applicable

- $174,070-$374,920 base annual salary range
