About the Role
Responsible for intaking new participants, assisting with SF Paratransit applications and renewals, conducting outreach, and managing the Child and Adult Care Food Program (CACFP).
Responsibilities
- Handles general inquiry calls and emails from potential participants and referral sources.
- Gathers information from potential participants to complete a detailed intake form.
- Submits Medical Treatment Plans to physicians and follows up.
- Conducts in-person and virtual presentation outreach in the community.
- Assists participants in applying for and renewing San Francisco Paratransit Group Van Transportation Service.
- Assists with and maintains participant's Child and Adult Care Food Program (CACFP) eligibility annually.
- Maintains and inputs daily meal records.
- Maintains the participant's Adult Day Services Participant Agreement.
- Provides administrative support to the Director, including preparation of statistical reports and office administration.
- Performs other duties as assigned.
Requirements
- High school diploma with a minimum of two years of office experience.
- Basic skills and knowledge of Microsoft Office processing (Word, PowerPoint, Excel) in English and Chinese.
- Bilingual in English and Cantonese.
- Must have TB clearance within 12 months prior to employment and maintain clearance annually thereafter.
- Must acquire CPR and First Aid certificates within 6 months of employment and maintain current certificates thereafter.
