About the Role
The Housing Case Manager (HCM) provides comprehensive Case Management services and care coordination to residents living with HIV and/or co-occurring conditions. The role focuses on ensuring housing stability by promoting independent living, financial management, and health and wellness.
Responsibilities
- Provide case management services including supportive counseling, motivational interviewing, crisis intervention, care coordination, linkage and referral, assessment, and service plan development.
- Conduct monthly home visits to clients' living spaces.
- Document all services and interventions provided according to funder, regulatory, industry, and internal standards.
- Ensure HIPAA forms and income verification documents are uploaded into client files.
- Create comprehensive initial assessments and service plans within 30 days of enrollment.
- Reassess and update service plans every 90 days.
- Provide referrals and follow-up for primary care, substance use services, nutrition, clothing, mental health services, and financial management.
- Maintain meaningful engagement with residents using motivational interviewing and supportive interventions.
- Promote participation in community activities for individual growth and social support.
- Connect and coordinate with internal departments such as ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, and Job-Training.
- Assist new clients with moving into apartments and transitioning from homelessness to stable housing.
- Develop strength-based individualized goals in collaboration with the client.
- Provide constructive feedback on goal participation during scheduled engagement.
- Accompany residents to facilitate access to external services as needed.
- Assess and discuss financial management with clients, including rent obligations, subsidy maintenance, utility payments, and nutritional needs.
- Complete apartment inspections and place work orders, collaborating with custodial and operations staff.
- Participate in weekly supervision meetings prepared.
- Complete all required reports and paperwork by prescribed deadlines.
- Demonstrate excellent customer service with clients, staff, vendors, and stakeholders.
- Coordinate care, including case conferencing, with residents' internal and external service providers.
- Participate in case conferences as part of a multidisciplinary team.
- Perform other responsibilities and duties as assigned.
- Participate in advocacy and direct actions to end homelessness and HIV/AIDS.
Requirements
- Bachelor's degree in social work, psychology, counseling, or a related field preferred; Master's Degree a plus.
- Minimum of three years of relevant experience in a social services setting working with individuals experiencing homelessness, mental health issues, and/or substance use.
- Ability to prioritize and multi-task.
- Experience with and proven commitment to working with low-income and marginalized communities, individuals engaging in substance use, and those with multiple chronic medical and/or behavioral health conditions.
- Ability to document in various electronic systems.
- Dependable and flexible.
- Able to maintain a high level of client confidentiality.
- Excellent internal and external customer service skills.
- Strong interpersonal skills.
- Good communication skills, both verbally and in writing.
- Proficient in Microsoft Outlook, Word, and Excel.
- Ability to work with colleagues to solve problems.
- Familiarity with Harm Reduction practices.
- Must be present in the program site 100% of the time.
- Working knowledge of NYC mass transportation systems (subway and bus) is required.
- Occasionally expected to de-escalate tense, even hostile, client situations.

- $24/hr - $31.25/hr commensurate with experience
