About the Role
The Supportive Housing Specialist will assist tenants in maintaining a stable community living environment by providing necessary linkages and support.
Responsibilities
- Assist tenants in securing safe and affordable housing.
- Support tenants with establishing and maintaining utilities.
- Assist with purchasing furniture and household items.
- Initiate monthly contact with tenants to review living environment and monitor mental health.
- Document all visits and client contacts in tenant records.
- Practice person-centered approaches and demonstrate a positive demeanor.
- Provide regular ongoing support to tenants and their service providers.
- Assist tenants with housing-related disputes with landlords.
- Provide teaching, monitoring, and recovery-oriented supports for independent living.
- Ensure tenants secure and maintain all entitlements.
- Collaborate with tenants and their support teams to create individualized housing support plans.
- Provide transportation to tenants as necessary.
- Ensure tenants maintain required clinical services and other linkages (medical, dental, vocational).
- Ensure all new tenants apply for HUD/Section 8 housing.
- Maintain adequate records, provider reports, and statistics.
- Coordinate tenant move-ins and move-outs.
- Perform other duties necessary for program operation.
Requirements
- Bachelor's degree or 4 years of mental health experience or equivalent.
- One year of experience in the human services field preferred.
- Clean NYS driving record.
- Use of personal vehicle is required.
- Must be at least 18 years of age.
