About the Role
Patient Navigators partner with the Care Team to provide patients with the information they need to approach their treatment cycle with confidence, simplifying a difficult and confusing process to ensure a seamless transition to the nursing team for cycle management.
Responsibilities
- Generate journey maps and calendars based on treatments plans
- Facilitate, track and document completion of tests and diagnostic procedures
- Develop trust and positive rapport with all new patients
- Explain complex medical process and terms in a digestible manger
- Monitor patient compliance and readiness for treatment
- Scheduling appointments
