About the Role
This position supports Housing Works' mission by connecting clients to programming through an efficient and friendly customer experience. The Community Liaison acts as an extender to the Account Management team, providing a warm welcome to clients. The ideal candidate is outgoing, personable, articulate, organized, and persistent in connecting individuals to care, ensuring an unparalleled and excellent experience at the Housing Works Community Health Center. Sensitivity to gender, race, ethnicity, socio-economic status, religion, age, sexual orientation, or other special needs is crucial for effective communication with staff and clients. The role requires upholding and advancing the values, ethics, knowledge, and mission of Housing Works.
Responsibilities
- Maintain knowledge of Housing Works' programming, screening tactics, and eligibility requirements, and articulate benefits to eligible clients.
- Assist in enrolling clients or registering patients in programming and help clients access entitlements, health insurance, and benefits if barriers exist.
- Coordinate client transportation and ensure client adherence to initial visits and appointments.
- Navigate new clients/patients throughout the Health Center site on their appointment day and deliver courtesy lunches.
- Provide follow-up support to ensure client/patient adherence to referrals, including follow-up calls and/or emails to Partner/Case Manager.
- Document and critically think through possible solutions for customer service issues.
- Assist the team in organizing outreach efforts, promotional materials, and producing data reports to improve partnership quality.
- Meet documentation and reporting requirements weekly and monthly to track client engagement and enrollments.
- Participate in professional training and development opportunities, including required certifications.
- Actively participate as a member of the Community Partnership team in weekly huddles, monthly meetings, and community events as needed.
Requirements
- High School Degree; Associate's or Bachelor's degree preferred or in pursuit.
- One to two years of experience in a community-based setting, including health services, as a client/patient navigator, outreach worker, or similar role.
- Demonstrated customer service skills, including problem-solving.
- Organized and able to handle multiple tasks in a fast-paced environment.
- Able to work independently while also supporting the team.
- Excellent verbal and oral communication skills, including phone etiquette and concise emails.
- Exceptional interpersonal skills; able to conduct oneself in a diplomatic and professional manner.
- Accurate, precise, and timely in reporting and managing numerous administrative tasks.
