About the Role

Provide comprehensive case management services to mentally ill, homeless adults, assisting with benefits, treatment referrals, independent living skills, and housing placement. Facilitate psychoeducational groups and generate progress notes and statistical reports.

Responsibilities

  • Provide comprehensive case management services to mentally ill, homeless adults.
  • Assist with obtaining benefits.
  • Provide referrals for treatment.
  • Monitor hygiene and teach independent living skills.
  • Provide escorts to and advocacy for entitlements and housing appointments.
  • Assist with housing placement.
  • Facilitate weekly psychoeducational groups.
  • Generate ongoing progress and group notes.
  • Generate monthly statistical reports.
  • Participate in clinical team, case conference, and other staff meetings.
  • Assist with program staff coverage as needed.
  • Provide emergency first aid/CPR assistance when needed.
  • Perform additional related duties as assigned.

Requirements

  • High School Diploma/GED required.
  • Bachelors degree preferred.
  • Masters degree ideal.
  • Bilingual Spanish strongly preferred, but not required.
  • Excellent communication and writing skills required.
  • Computer proficiency required.
  • Experience working in residential treatment.
  • Experience working with MICA populations.
  • Experience working with homeless populations.
  • Knowledge of HRA application systems a plus.
  • Knowledge of NY/NY application systems a plus.
  • CPR training certification or willingness to take training class in CPR.
  • Vaccination preferred but not required.
