About the Role

Responsibilities

  • Identify and interview families to assess needs and determine eligibility for home visitation services.
  • Provide education to families regarding available resources in Steuben County.
  • Nurture existing partnerships with referral sources and build new relationships to increase referrals.
  • Coordinate services with various agencies and referral sources.
  • Distribute and collect screening tools from referral sources.
  • Maintain positive working relationships with referral sources.
  • Act as a representative to the community by providing occasional presentations.
  • Conduct and document parent survey assessments.
  • Present appropriate families to the Program Supervisor.
  • Ensure a seamless transition of families to the Family Support Specialist.
  • Maintain knowledge of existing referral sources and share information with families.
  • Provide short-term direct services, such as family advocacy.
  • Record parent survey assessment narratives in the HFNY Management Information System.
  • Complete all required documentation, reports, and HFNY State Forms.
  • Complete Healthy Families Parent Survey Community Outreach Core Training and subsequent trainings.
  • Participate in quality assurance visits with Prevent Child Abuse New York and HFNY Program Contract Manager.
  • Participate in regular staff meetings and meetings with supervisor.
  • Complete other duties as assigned.

Requirements

  • Possession of a valid NYS Driver’s license and access to a reliable vehicle.
  • At least one year of experience working with families.
  • Experience working with families of diverse cultures with cultural sensitivity, compassion, and empathy.
  • Strong interpersonal skills with the ability to relate to people with respect.
  • Adherence to all applicable federal and state laws and regulations.
  • Excellent verbal and written communication skills.
  • Ability to prioritize assignments, plan, and complete work projects with minimal direction.
  • Exceptional time management skills and ability to meet deadlines.
  • Willingness to foster agency, department, and program-wide cooperation and teamwork.
  • Ability to maintain absolute confidentiality of all records.
  • Proficiency in Windows, Microsoft Office, and multiple online software platforms.
  • Ability to analyze and interpret data and handle problem resolution.
  • A driver's record considered acceptable by the agency and insurance carrier.
  • Continuous use of a reliable, registered, and insured vehicle.
  • Demonstrated commitment to the Agency Mission Statement.
  • Ability to provide effective, equitable, understandable, and respectful quality care and services responsive to diverse cultural needs.
  • Ability to demonstrate behaviors and attitudes that support organizational mission, philosophy, and policies.
  • Ability to work in a cooperative and helpful manner with all individuals.
  • Successful completion of a background check.
