About the Role
The Medication Case Manager provides various support services to residents with psychiatric disabilities in the Apartment Treatment Program, reporting to the Program Supervisor.
Responsibilities
- Provide oversight and monitoring of client medications at the homes of identified clients.
- Link residents with appropriate resources.
- Engage in ongoing assessment of client needs to determine frequency of medication monitoring.
- Provide education and resource information regarding medication and medical and mental health disorders.
- Prepare clients to monitor and take their medications independently.
- Adhere to Options’ medication policies and procedures.
- Complete timely submission of medication management progress notes, assessments and other relevant documentation.
- Conduct overall assessment of client welfare and appropriate intervention and follow-up.
- Participate in on-going communication with medication prescribers, service providers, Case managers, and collateral contacts.
- Provide training on medication management and supervision for Licensed Program staff.
- Travel to apartment sites within the Mental Health Licensed Program using personal or agency vehicle.
- Assist with medication count supervision across apartment treatment program sites.
- Meet with the Program Supervisor at least once per week to discuss client progress.
- Maintain a case load and perform the duties of a full-time residential counselor as needed.
- Perform any other relevant tasks as assigned by management.
Requirements
- Associate's degree and 2 years of qualifying experience or a Bachelor's degree and 1 year of qualifying experience.
- Valid driver’s license, safe driving record, valid auto insurance and access to a vehicle is required.
- Qualifying experience: Verifiable full or part time case management or case work with persons with a history of mental illness, homelessness, or chemical dependence.
- An alternative combination of education, credentials, and experience may be substituted for a degree.
