About the Role

The Case Manager provides direct casework services to clients/residents, assessing needs, developing service plans, and ensuring client progress.

Responsibilities

  • Arrange meetings with newly assigned clients to assess basic needs and develop a service plan.
  • Conduct psychosocial assessments to determine individual client needs.
  • Carry an assigned caseload of residents/clients.
  • Perform concrete casework requirements and referrals.
  • Complete all required paperwork, case management notes, referral sheets, and statistical requirements in a timely manner.
  • Resolve client incident reports and complaint reports.
  • Participate in consumer hearings and discharges.
  • Track client progress, report on each case, and analyze service plan results.
  • Develop and implement treatment plans, track progress, and evaluate outcomes.
  • Make referrals to appropriate resources and determine eligibility for referrals or information.
  • Participate in weekly supervision meetings for case and administrative consultation.
  • Handle emergencies, arranging for medical care or other services.
  • Attend agency or professional group seminars for ongoing development.
  • Attend staff meetings as requested.
  • Adhere to all policies and procedures.
  • Maintain confidentiality of all information.

Requirements

  • Bachelor’s Degree in Social work or related field
  • Certification in First Aid, CPR
  • Proficient in computer software programs (Word, Excel, Power Point, CARES, etc.)
  • Excellent verbal and written communication skills.
  • Ability to read and comprehend simple instructions, short correspondence, and memos.
  • Ability to write simple correspondence.
  • Ability to present information in one-on-one and small group situations.
  • Ability to climb and ambulate stairs (approx. 10 flights).
  • Ability to stoop, kneel, crouch, reach, walk, stand, lift, finger, grasp, talk, hear, and feel.
  • Ability to maintain close visual acuity for data analysis, computer use, and reading.
