About the Role

The Jericho Project Case Manager will provide residential and long-term support services to homeless and at-risk adults transitioning into permanent housing, managing a caseload of approximately 20-25 individuals. This role involves frequent tenant interaction, including face-to-face meetings, phone calls, and electronic communication with colleagues.

Responsibilities

• Provide direct case management services, including mental health and substance use assessments, service plan development, and assistance with public benefits.
• Utilize a person-centered approach with evidence-based models like Motivational Interviewing, Critical Time Intervention, and Harm Reduction.
• Maintain accurate tenant files and electronic health records.
• Coordinate and document clinical case management and psychosocial services, including developing and reviewing Individual Service Plans.
• Serve as a liaison between tenants and community providers.
• Facilitate groups focused on life skills, education, support, relapse prevention, and mental health counseling.
• Complete referrals to external medical, mental health, and other providers.
• Collaborate with Workforce Opportunities Staff to assist tenants with employment goals.
• Ensure compliance with organizational and funder regulations and documentation requirements.
• Coordinate aftercare services and follow-up contacts for post-discharge updates.
• Conduct outreach and engagement, including apartment visits and inspections.
• Participate in community development efforts and support partnerships with other providers.
• Work with Property Management to assist tenants with lease terms, rent payments, and avoiding nuisance behaviors; mediate landlord-tenant disputes.
• Utilize trauma-informed interventions and culturally responsive engagement techniques.
• Participate in community initiatives to support Jericho Project’s mission.
• Provide crisis intervention as needed.
• Provide services respectfully, considering cultural differences.
• Carry out other duties as assigned by leadership.

Requirements

• A Bachelor’s degree in human services or a related field.
• Strong organizational, writing, communication, and computer skills.
• Relevant experience working with low-income or homeless populations.
• Candidates must be self-starters with the skills and energy to work within the community.
• Ability to lift up to 50 lbs.
• Ability to travel to various sites.
• Ability to speak, hear, see, and write.
• Ability to use a computer and related software programs.
• Local and/or national travel required, potentially including weekday/weekend commitments.
