About the Role

The Senior Case Manager will provide rapid rehousing services to high-risk veterans, young adults, and families, managing a caseload of approximately 10-12 participants. This role involves frequent communication with participants and oversight of the case management team, including documentation review, financial request approval, and staff onboarding.

Responsibilities

  • Utilize Motivational Interviewing and Harm Reduction techniques to assist homeless individuals and families in resolving housing crises and promoting long-term stability.
  • Conduct assessments and program intakes for rapid re-housing services, ensuring data accuracy and completeness.
  • Update case notes in the electronic case management system promptly.
  • Make referrals for mental health services and complete psychosocial evaluations.
  • Act as a liaison to submit supportive housing applications.
  • Assist in the development and facilitation of workshops and groups on independent living skills, healthy relationships, and mental health.
  • Conduct intakes and assist in the orientation process for new program participants.
  • Provide direct case management services in the office and through field/home visits.
  • Develop and update housing stability plans, identifying participant strengths, needs, abilities, and preferences.
  • Assist participants in overcoming barriers to housing stability, including increasing income and benefits.
  • Provide hands-on assistance to engage high-risk participants in finding suitable housing, including accompanying them to viewings and appointments.
  • Advocate on behalf of participants with landlords and brokers to resolve issues and maintain housing.
  • Collaborate with community housing specialists to identify suitable housing options.
  • Assess participants' housing readiness and determine the appropriate level of housing.
  • Conduct housing re-inspections and mediate landlord-tenant disputes.
  • Serve as a point-of-contact for landlords and maintain positive landlord relationships.
  • Conduct bi-monthly face-to-face contact with participants and quarterly home visits.
  • Coordinate with the Workforce Opportunities team to connect participants with employment and educational assistance.
  • Identify and make referrals to community resources for services such as preventive care, long-term case management, benefits assistance, mental and physical healthcare, and more.
  • Assist with household budgeting and provide referrals for financial counseling.
  • Assess participants' need for financial assistance and submit completed requests.
  • Provide crisis intervention and skill-building as needed.
  • Utilize the electronic case management system and maintain accurate client files.
  • Assign trainings to case management staff and ensure completion of required training.
  • Participate in community initiatives to support the organization's mission.
  • Participate in and coordinate case conferences, staff meetings, and training.
  • Perform other duties as assigned by leadership.

Requirements

  • Bachelors’ Degree in social work or a similar field.
  • Experience working in homelessness, substance abuse, mental health, and/or trauma settings is preferred.
  • Experience working with veterans/military, young adults, or LGBTQ populations is preferred.
  • Knowledge of NYC resources is preferred.
  • Strong organizational, writing, communication, and computer skills.
  • Ability to perform fast-paced case management and some fieldwork.
  • Must be a self-starter with the skills and energy to work with high-need participants in the community.
  • Strong engagement and interpersonal skills.
  • Ability to lift up to 50 lbs.
  • Ability to travel to various sites.
  • Ability to speak, hear, see, and write.
  • Ability to use a computer and related software programs.
  • Local and/or national travel required.
  • May require weekday/weekend commitments for business projects or deliverables.
