About the Role
Mindify Wellness and Care is seeking a compassionate and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) to join our growing behavioral health team. Responsibilities
- Conduct comprehensive psychiatric evaluations and ongoing assessments
- Diagnose behavioral health conditions using DSM-5-TR criteria
- Develop individualized treatment plans
- Prescribe, manage, and monitor psychiatric medications
- Conduct risk assessments and crisis evaluations when clinically indicated
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and families regarding diagnoses, medications, and treatment options
- Support patient engagement and treatment adherence
- Provide therapeutic interventions within scope of practice
- Collaborate with therapists, social workers, primary care providers, and other members of the treatment team
- Participate in case consultations and treatment planning discussions
- Coordinate referrals and support continuity of care
- Complete timely and accurate documentation within the EMR
- Follow organizational policies, clinical standards, and applicable regulations
- Maintain compliance with HIPAA and professional practice standards
Requirements
- Master's or Doctoral Degree from an accredited Psychiatric Mental Health Nurse Practitioner program
- Active New York State RN License
- Active New York State Nurse Practitioner License
- Board Certification as a PMHNP-BC
- Active DEA Registration with prescriptive authority
- Experience in outpatient behavioral health settings
- Telehealth experience
- Experience serving diverse patient populations
- Bilingual candidates are strongly encouraged to apply, especially those fluent in Spanish and Russian

- $60-90 per visit for Initial Psychiatric Evaluations
- $40-60 per visit for Follow-Up Medication Management Visits
