About the Role

The Physical Therapist (PT) role is core to Adaptive Home Health's mission to dramatically improve patient access to home health care. You will restore function, keep patients safe at home, and drive real outcomes. Adaptive Home Health also builds technology to support field clinicians, reducing documentation burden and simplifying scheduling.

Responsibilities

  • Own the caseload as a case manager, accountable for the patient’s therapy plan, progression, and outcomes.
  • Complete comprehensive in-home assessments and deliver skilled PT visits.
  • Build and execute a clear plan of care including gait/balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
  • Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
  • Drive multidisciplinary coordination by communicating changes in condition and aligning on orders.
  • Manage treatment and outcomes directly, including PTA supervision where appropriate.
  • Coach patients and caregivers with practical home programs.
  • Document promptly and thoroughly in Homecare Homebase (HCHB).

Requirements

  • Active Texas PT license in good standing
  • Strong clinical judgment and comfort working independently in the field
  • Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
  • Strong communication and documentation skills
  • Reliable follow-through on orders, frequencies, and patient goals
  • Current driver’s license and comfort with daily travel across assigned territory
  • Able to carry/transport basic therapy equipment as needed
  • Competency in core home health PT scope including safety-first mobility work, strengthening, functional mobility, home safety assessment/modification education, and patient/caregiver coaching with practical home programs.
  • Prior home health experience is a plus.
  • Homecare Homebase (HCHB) experience or similar home health EMR is a plus.
  • Deeper clinical experience in high-signal PT populations is a plus.
  • Experience supervising/mentoring PTAs and collaborating on care plans is a plus.
  • Strong organization and time management skills are a plus.
