About the Role

The RN Care Coordinator is responsible for the continuity of patient care across the Amboy Medical Practice and supports the clinical and quality performance goals of the practice. The RN Care Coordinator routinely assesses patients for readiness to learn, barriers to care, social determinants of health and other psychosocial behaviors that impact care. The RN Care Coordinator is a resource to providers and clinical staff and ensures that the practice understands and meets the needs of its patients through pre-visit planning, daily huddles, referral and test tracking a care coordination services.

Responsibilities

  • Assesses patients for readiness to learn, barriers to care, social determinants of health and other psychosocial behaviors that impact care.
  • Serves as a resource to providers and clinical staff.
  • Ensures the practice understands and meets patient needs through pre-visit planning, daily huddles, referral and test tracking, and care coordination services.

Requirements

  • NYS RN license
  • Current BLS
  • Minimum two or more years of ambulatory, clinic, or physician practice care experience required, preferably in a recognized Patient Centered Medical Home or similarly recognized program.
