About the Role

Assists in navigating the home and community-based long-term care system by providing assessment, information, referral, and assistance to older adults, adults with disabilities, and caregivers. This role involves follow-up to ensure service delivery and collecting/updating community resource information.

Responsibilities

• Conducts basic needs assessments for older adults and disabled adults to provide relevant information and/or referrals.
• Assists consumers in accessing services and advocates on their behalf for non-casework level assistance.
• Provides follow-up with consumers and agencies to confirm service receipt.
• Documents services according to program protocol and submits monthly logs and reports.
• Develops and implements outreach strategies in collaboration with the Citywide I&A Supervisor.
• Assists the Social Services Department in community outreach to promote programs.
• Establishes and maintains positive working relationships with key personnel in other agencies.
• Participates in assigned meetings and professional development training.
• Works flexibly across different locations as needed.
• Performs other duties as assigned.

Requirements

• Bachelor’s degree in human services or high school graduate with two years of related work experience.
• Experience providing health, social, and/or aging services, particularly with diverse communities.
• Knowledge of community resources.
• Sensitivity to the needs of the elderly and disabled adults from diverse ethnic and minority cultures.
• Good problem-solving skills and ability to follow through.
• Excellent verbal and written communication skills.
• Excellent interpersonal skills.
• Proficiency in MS Office and the Internet.
• Ability to travel via public transportation or personal vehicle.
• Must possess and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage if driving.
