About the Role

The Mental Health Counselor provides psychosocial assessments, collaborative treatment planning, engagement, referrals, and evidence-based clinical interventions for individuals with mental health and co-occurring substance use issues.

Responsibilities

  • Conducts initial psychosocial assessments and provides initial treatment recommendations.
  • Collaboratively develops personalized treatment plans with patients, emphasizing evidence-based practice and harm reduction.
  • Provides resources and referrals to facilitate patient treatment goals.
  • Provides psychotherapy and crisis intervention to individuals and families dealing with mental health, substance use disorders, chronic medical conditions (including HIV, HCV), and issues related to sexual orientation and gender identity.
  • Adheres to program expectations for productivity, documentation timelines and standards, and overall quality of patient care.

Requirements

  • Master of Social Work or other related field.
  • Two (2) years of related experience.
  • LMSW or LCSW preferred.
  • Ability to communicate clearly and professionally in writing and verbally.
  • Familiarity with computers and MS Office software (Outlook, Word).
  • Effective time management and organization skills.
  • Awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential.
  • Working knowledge utilizing electronic medical records software (Medent) strongly preferred.
  • Knowledge of Diagnostic and Statistical Manual of Mental Disorders.
