About the Role
Project FIND provides services and support to low- and moderate-income and homeless older adults. The Social Worker will provide case management, counseling, and support services to residents aged 62 and older at the Park 79 affordable housing location.
Responsibilities
- Conduct intake and assessment screening, assessing mental health and physical health needs and support, and eligibility for benefits and entitlements.
- Conduct home, office, and community visits to address unmet needs.
- Implement constructive short and long-term interventions through counseling, education, long-term planning, assistance with benefits and entitlements, and linkage and referral to community-based providers.
- Provide orientation, support, and services to assist tenants in remaining permanently housed in a safe environment.
- Assist with income affidavits, housing applications, recertifications, and collecting rent as needed, in collaboration with Property Management.
- Interface with the Property Management team to resolve ongoing concerns or issues related to the residence, including tenant and facility concerns.
- Develop and create training events to educate clients on topics such as self-care, health and wellness, and legal planning.
- Lead one or more support groups, providing emotional support and psychoeducation about aging in place, health and wellness, and self-care.
- Develop and create community building events and opportunities to facilitate activity and socialization.
- Provide conflict resolution and engage in tenant concerns requiring enhanced intervention.
- Provide timely documentation and maintenance of client files and progress notes.
- May provide supervision, training, and support to paraprofessional staff, including students.
- Perform other related tasks as requested or assigned.
Requirements
- Master’s degree in social work or related field of study, or commensurate experience equaling a minimum of four years’ experience in the human services sector.
- LMSW or LCSW strongly preferred with at least 2 years’ relevant experience in social service.
- Experience working with older adult populations and families preferred.
- Bilingual Chinese or Spanish language fluency preferred.
- Strong computer skills including Microsoft Office.
- Ability to provide accurate and timely documentation.
- Time management skills to coordinate multiple responsibilities and services.
- Ability to work independently and as part of a team, in both a home and office environment.
- Ability to maintain client and agency confidentiality.
- Excellent interpersonal, public speaking, and written communication skills.
- Must be able to communicate effectively and appropriately with other team members, community agencies, and clients with diverse opinions, values, characteristics, and cultures.
