About the Role

Little Flower Children and Family Services of New York is seeking conscientious and caring individuals to join their team. This role offers a rewarding career where you can make a difference by strengthening families and supporting the well-being of children and adults with developmental disabilities. The position focuses on providing Home and Community Based Services (HCBS) and Children and Family Treatment and Support Services (CFTSS) to meet behavioral health needs and prevent the need for more restrictive services.

Responsibilities

  • Participation in the development and ongoing monitoring of the child’s Treatment Plan.
  • Provide direct services to child & family on caseload.
  • Services are provided throughout Long Island.
  • Documentation of all service provision and contacts.
  • Attendance at all meetings deemed necessary.
  • Coordination of care with other providers and/or collaterals to assist child in obtaining their optimal functioning level within the community setting.
  • Currently the position is remote and if needed will report to the Wading River office.
  • Attend all required trainings.

Requirements

  • Bachelor’s degree in social work, Psychology or a related field, plus four years of related experience required.
  • Master’s degree in social work, Psychology or a related field preferred, plus one year of related experience required.
  • Excellent writing, communication and word processing skills required.
  • Ability to travel to various communities.
  • Ability to work a flexible schedule including weekends and evenings.
  • Possession of a valid NYS driver’s license is required.
