About the Role

CHI is seeking a full-time Case Manager for its Commack location. The role involves ensuring program participants receive services to promote personal and economic independence, secure and maintain permanent housing, and live in well-maintained units.

Responsibilities

  • Adhere to and enforce program regulations, policies, and procedures.
  • Ensure clients receive quality services that meet program requirements.
  • Assist clients in developing an Independent Living Plan (ILP) by identifying needs and goals.
  • Evaluate client progress on short- and long-term goals.
  • Provide information, referrals, counseling, crisis intervention, direct service, and advocacy.
  • Develop and maintain a resource network for client referrals.
  • Collaborate with DSS and other service providers to assist clients with ILP and program requirements.
  • Assist clients with direct services including budgeting, resume development, and ADL skills.
  • Conduct face-to-face meetings with clients.
  • Regularly inspect living spaces to assess client ADL needs and program compliance.
  • Document and limit improper use of agency resources by clients.
  • Evaluate and report physical plant needs of housing units and common areas to supervisor.
  • Complete discharge plans with clients.
  • Submit all internal and external reports in a timely and correct manner.
  • Maintain charts and files in an orderly fashion.

Requirements

  • Two years of experience working in a related field such as homelessness or housing preferred.
  • Excellent verbal and written communication skills.
  • Proficiency in English required, bi-lingual (Spanish) is a plus.
  • Basic competency in MS Windows, MS Office, and internet usage.
  • Ability to formulate simple financial budgets.
  • Ability to tour property, walk distances, and climb stairs.
  • Ability to prioritize, make appropriate decisions, and judgment calls.
  • Ability to relate to the underserved, homeless, and disabled populations.
  • Ability to work independently and as part of a team.
  • Highly organized.

Salary/Compensations

  • $24.04 Hourly (Bachelor's degree)
  • $26.45 Hourly (Master's degree)
  • $55,000.00 Hourly Range
