Responsibilities
- Participates in programs and procedures designed to promote the appropriate, safe, and effective use of medications for Thompson Health patients and residents.
- Provides professional services and support in medication distribution and compounding.
- Provides professional services and support in drug information.
- Provides professional services and support in education and resource materials.
- Provides professional services and support in formulary management/drug usage.
- Provides professional services and support in adverse drug reactions.
- Provides professional services and support in medication error prevention.
- Provides professional services and support in nursing/pharmacy liaison.
- Provides professional services and support in clinical computer applications.
- Provides professional services and support in medication use evaluation.
- Provides professional services and support in program development and implementation.
- Lives the CARES values at all times.
Requirements
- Current New York State Pharmacist registration.
- Accredited continuing education as required/defined by New York State.
- Minimum of one year of ambulatory care pharmacy experience preferred.
- Demonstrates skill in daily communication including effectively gathering and disseminating information.
- Exhibits flexibility in decision-making and prioritizing assignments.
- Knowledgeable in proactively resolving problems, making recommendations, and implementing continuous quality improvements.
- Mature, energetic, and creative individual who demonstrates team-player qualities demanded by a rapidly changing health care environment.
- Possesses expertise in pharmaceutical care, organizational skills, and in managing daily workload expectations.
- Must show a willingness to learn, take risks, assume responsibility, and pursue excellence.
- Possesses, maintains, and enhances knowledge of all accrediting and/or regulatory agency standards (i.e. Joint Commission, DOH, etc.).
- Actively guards the confidentiality of sensitive information including but not limited to the patients, staff, and the health system.
- Demonstrates the knowledge & skills necessary to provide care appropriate to the age of the patient.
- Possesses an in-depth knowledge of medications and their uses.
- Demonstrates the CARES Values and Standards of Cooperation at all times.
