About the Role

As a Client Care Manager, you will manage your own portfolio of clients, ensuring they receive the highest standards of care and support. You will work as part of a supportive team alongside two other Client Care Managers and the Head of Client Services, with support from a Medication Management Administrator. Your client portfolio will be based within a defined geographical area to minimize travel time. You will oversee all aspects of client care, ensuring it is safe, effective, compliant, and centered around individual needs.

Responsibilities

  • Manage own client portfolio and maintain strong, professional relationships with clients and their families.
  • Monitor care quality and delivery, ensuring it is person-centred and meets individual client needs.
  • Ensure care delivery aligns with relevant legislation, CQC requirements, and company policies.
  • Review and update care plans as client needs change.
  • Support clients during hospital admission and discharge, especially when care needs change.
  • Liaise with multidisciplinary teams, discharge planners, ward staff, and other healthcare professionals for safe hospital discharge.
  • Collaborate with GPs, social workers, pharmacies, district nurses, and community services for coordinated client support.
  • Identify changes in client needs and make appropriate referrals to community services and other support sources.
  • Work closely with the Business Development Manager on initial client assessments, risk assessments, care plans, and medication requirements.
  • Provide professional advice, guidance, and reassurance to clients and their families.
  • Ensure accurate and timely record maintenance.
  • Support the team in maintaining high standards of care, compliance, and customer service.

Requirements

  • At least 2 years of experience in a care management role.
  • Experience writing and reviewing care plans.
  • Good knowledge of CQC regulations and the Key Lines of Enquiry (KLOEs).
  • Experience using people management or care management systems.
  • Excellent communication and interpersonal skills.
  • Strong organisational, time management, and prioritisation skills.
  • A professional and positive approach.
  • Ability to work independently and contribute effectively to a team.
  • Flexibility to meet business needs.
  • Full UK driving licence and access to own vehicle for client visits.
  • Local to New Malden or within reasonable travelling distance.
  • Subject to an Enhanced DBS check.
