About the Role
The Community Care Specialist provides intensive, one-on-one support to youth and families through recreational, educational, cultural, and social activities. This role assists in supervising, counseling, teaching, and supporting children, families, and foster families within the Family Visitation and Empowerment Program (FVEP), a trauma-informed reunification program focused on maintaining family connections for children in foster care.
Responsibilities
- Provide individual treatment and supportive services to youth and families, including role modeling, homework assistance, and participation in educational activities as identified in the treatment plan.
- Participate in the development of individual service plans.
- Conduct family team meetings to discuss ongoing decision-making processes.
- Act as an advocate for youth with community agencies and facilitate linkages to resources.
- Maintain regular communication with guardians, foster parents, and biological parents.
- Assist with youth supervision and provide transportation for youth and families.
- Collaborate with service providers such as DSS and Mental Health Clinics for optimal service delivery.
- Document the status and progress of all goals, including educational, vocational, social/emotional, behavioral, and personal matters.
- Complete all reports in a timely manner.
Requirements
- Minimum High School diploma required; Associate’s degree preferred.
- Minimum 1 year of experience working with youth and families, human services, social services, child welfare, or in a not-for-profit setting is preferred.
- Must be able to work a flexible schedule including various hours, nights, and weekends.
- Must possess a Valid Driver’s License with an acceptable driving history.
- Must have a reliable vehicle.
- Ability to work with economically and culturally diverse populations.
