About the Role

As a Social Worker with our Hospital Discharge Team, you will make a powerful contribution to Westminster's success. You will work closely with health colleagues within an acute setting and be an active member of a ward multi-disciplinary team. Your responsibilities will include undertaking assessments of need, developing support plans for patient discharge, conducting safeguarding investigations, and performing Mental Capacity Act assessments. You will also work with carers and other professionals to minimize and manage risk while helping patients maintain their chosen lifestyle. This role involves working five days a week, including Saturdays, Sundays, and Bank Holidays, offering an alternative 5-day work pattern.

Responsibilities

  • Undertake assessments of need
  • Develop support plans that help patients with their discharge from hospital
  • Undertake safeguarding investigations
  • Perform Mental Capacity Act assessments
  • Work with carers and other professionals to minimize and manage risk
  • Help each patient to maintain their chosen lifestyle
  • Offer advice and information to colleagues and other stakeholders on the role of the social worker and the service provided
  • Join the urgent response and duty system
  • Participate in developments to the service
  • Ensure professional registration and DBS are kept up to date
  • Be responsible for own CPD
  • Participate in appraisals, audits, evaluations, reflective practice and team meetings
  • Work in line with all policies and procedures including health and safety, data and case recording, supervision, data protection and information governance
  • Inform manager of any significant events relating to the service
  • Undertake best interest and practice teacher training
  • Take part in Trusted Assessor training to prescribe simple equipment and assistive technology
  • Adopt an inter-disciplinary approach to work
  • Collaborate with all partners in order to achieve best outcomes

Requirements

  • CQSW/Diploma in Social Work, Degree in Social Work or its equivalent
  • Registered as a member of Social Work England
  • Post-qualifying experience in a relevant Local Authority or Health setting
  • Well-developed knowledge of relevant legislation
  • Good understanding of the principles of personalisation, independence and choice
  • Experience of working with older or physically disabled people in a Social Services Duty System
  • Experience working in a multi-disciplinary way
  • Experience working in an acute setting
  • Experience of assessment work
  • Ability to work independently
  • Ability to use initiative
  • Ability to prioritise workloads and meet deadlines
  • Ability to negotiate with other agencies on behalf of the Commissioning Authority and service users
  • Pro-active worker with the skills to pre-empt, prevent and manage any issues that arise
  • Good case recording and assessment skills
  • Understanding of budgets and financial controls
  • Ability to complete robust risk assessments
