About the Role

The Service Coordinator (SC) provides case management to participants and prospective participants of the Home and Community Based Service/Traumatic Brain Injury Waiver Program of New York State. The SC coordinates and monitors all services in the participant’s Service Plan, aiming to increase independence, productivity, and community integration while maintaining health and welfare.

Responsibilities

  • Prepare and submit all required documentation and semi-annual service reports in a timely manner, aligning with Department of Health (DOH) and waiver program guidelines.
  • Ensure reports are shared with appropriate staff and agencies.
  • Maintain accurate and well-organized records for each waiver participant, adhering to Schofield Home Care’s policies and regulatory standards.
  • Conduct monthly in-person visits with participants for support, updates, and service satisfaction assessment.
  • Maintain thorough documentation of all participant contact, including meetings, phone calls, and emails.
  • Organize and lead Clinical Team Meetings based on each participant’s approved Service Plan, ensuring representation from all service providers.
  • Protect participant confidentiality at all times, following HIPAA regulations and internal privacy practices.
  • Monitor and manage supplies and equipment used for service coordination activities.
  • Undertake additional duties as needed to support program goals and participant needs.

Requirements

  • Must possess a valid driver’s license.
  • Must have a car available for use.
  • Must possess good moral character, patience, and compassion for the chronically ill or disabled.

Experience Level

  • Minimum of one (1) year of experience providing service coordination for individuals with disabilities (with specific degrees/certifications).
  • At least two (2) years of experience in service coordination for individuals with disabilities (with a Bachelor's degree).
  • At least three (3) years of experience in service coordination for individuals with disabilities (with an Associate's degree).
  • Minimum of four (4) years of relevant experience in service coordination (with a High School Diploma or GED).
  • Previous successful experience as a Regional Resource Development Specialist (RRDS).
