About the Role

The Care Coordinator facilitates the wraparound process for youth and families by coordinating and managing family-driven care necessary to meet the complex needs of youth experiencing behavioral and mental health difficulties. Wraparound services are individualized, community-based services that focus on the strengths and needs of the child and family.

Responsibilities

  • Conduct needs assessments and design Plans of Care for assigned families.
  • Authorize and connect families to appropriate service providers and community resources.
  • Request and manage funds to address specific client needs.
  • Conduct home visits and provide ongoing supportive guidance to youth/families.
  • Monitor the psychological and emotional wellbeing of youth/families.
  • Conduct school visits and monitor students’ attendance, behavior, and grades.
  • Create crisis plans and provide crisis management for youth and families.
  • Develop and maintain positive relationships with stakeholders and community service providers.
  • Engage and authorize community partners to meet needs outlined in the Plan of Care.
  • Exchange student information and problem-solve with school personnel.
  • Participate in school-based Child Study Teams.
  • Manage interdisciplinary teams assembled to support each family’s Plan of Care.
  • Facilitate regular meetings and communication among interdisciplinary team members.
  • Monitor the quality of services provided to youth/families.
  • Monitor and amend monthly team budgets.
  • Create and maintain paper case files in accordance with established timeframes and contractual requirements.
  • Create and maintain electronic files in the TCM database.
  • Enter and update case activity, progress notes, Plan of Care, financial, educational, juvenile justice, and court screens in TCM database.
  • Monitor monthly service provider notes.

Requirements

  • Master's degree in social work, counseling/guidance, or related field OR Bachelor’s degree in a related field with at least one year of experience working with youth and/or families.
  • Understanding of child and adolescent development.
  • Knowledge of family systems theory and application.
  • Knowledge of cognitive-behavioral therapy theory and application.
  • Knowledge of social skills assessment and intervention.
  • Basic working knowledge of Microsoft O365, MS Word, Excel, and Outlook.
  • Database management skills adequate to enter and retrieve data.
  • Demonstrated ability to work independently, self-directed, and with good judgment.
  • Ability to balance multiple and conflicting priorities.
  • Ability to create effective relationships with individuals of different cultural beliefs and lifestyles.
  • English-Spanish bilingual proficiency is strongly preferred.
  • Reliable transportation is required.
