About the Role

Join Humber River Health's dynamic team as a Registered Pharmacist, providing safe and effective medication preparation and distribution. Our Pharmacy department is critical to patient treatment outcomes, ensuring compliance with all standards. We are seeking a qualified, technology-comfortable, and team-oriented Registered Pharmacist to join our growing operations and deliver compassionate, patient-centered care.

Responsibilities

• Provide clinical pharmacy services to improve patient quality of life.
• Manage "drug use control" responsibilities, including order review, profile maintenance, IV admixture, TPN, pediatric and chemotherapy preparation, drug information, therapy monitoring, and adverse drug reaction reporting.
• Perform Computer Provider Order Entry (CPOE) and prescription verification in MEDITECH.
• Monitor clinical pharmacotherapy, check chemotherapy, and dispense selective medications.
• Serve as a preceptor or clinical tutor for pharmacy students and interns.
• Perform other duties as assigned.

Requirements

• Bachelor of Science in Pharmacy required; Pharm D is an asset.
• Current Part A registration with the Ontario College of Pharmacists required.
• Previous hospital experience in unit dose and/or IV additive is preferred.
• Hospital residency program or equivalent is an asset.
• Demonstrated ability to work independently and as part of a team.
• Demonstrated ability to prioritize a consistent and varied workload.
• Ability to direct and teach staff.
• Demonstrated written and verbal communication skills for effective interaction with patients, peers, healthcare providers, and volunteers.
• Demonstrated ability to identify, prevent, and resolve drug-related problems.
• Good understanding of change management, including identifying change needs and acting proactively.
• Effective negotiation and conflict management/resolution skills required.
• Excellent attendance and discipline-free record required.
• Understanding of equity, diversity, and inclusion principles as they relate to health practices is an asset.
