About the Role

The Safe Options Support (SOS) Care Manager will be a key member of a collaborative, multi-disciplinary team seeking to engage the unhoused population of Monroe County. The team's mission is to ensure the greatest level of independence and well-being for homeless and unstably housed individuals using the Critical Time Intervention model of care.

Responsibilities

  • Outreach and build relationships with displaced and unhoused persons, meeting participants at known locations and settings.
  • Make referrals to necessary care and support services utilizing warm handoffs.
  • Work closely with existing care teams of individuals being served.
  • Monitor engagement in services and housing stability through continued connection.
  • Administer assessments related to mental health, addiction, and social determinants of health.
  • Perform crisis management and safety planning using the Columbia Protocol and other best practices.
  • Complete all required documentation in accordance with program standards.

Requirements

  • Bachelor’s degree or higher, preferably in psychology, social work, sociology, or related field, or be a New York State Licensed Practical Nurse (LPN).
  • Past case management work experience will be considered in lieu of a Bachelor's degree.
  • Experience providing advocacy with homeless and/or precariously housed populations preferred but not required.
  • Experience as a provider and/or recipient of behavioral health, substance use, and/or homeless services.
  • Knowledge of homeless resources within the community.
  • Knowledge of treatment, rehabilitation, and community support programs.
  • Knowledge of community-based organizations and opportunities for continuing wellness development.
  • Knowledge of Intentional Peer Support models, best practices, and methods of implementation.
  • Awareness of personal and systemic issues that hinder access to care.
  • Bi-lingual in Spanish and English preferred.
