About the Role
Provides nursing care for residents by offering supportive and restorative care, teaching, and collaborating on resident service plans within the Assisted Living/Memory Care Unit.
Responsibilities
- Collects data based on age, needs, and abilities to contribute to resident care and service plans, participating in care planning meetings.
- Communicates and collaborates effectively with residents, families, colleagues, and the healthcare team to meet resident needs.
- Provides education, supervision, and counseling to residents and families regarding assisted living procedures and care needs.
- Documents accurate, pertinent, and comprehensive information in the client service record in a timely and concise manner.
- Conducts supervisory visits of resident care assistants, companions, and homemakers as required.
- Participates in educational programs, inservices, meetings, and workshops to enhance professional skills and knowledge.
- Oversees medication management, including ordering, storing, reconciling, and preparing medications for administration, under supervision and in collaboration with physicians, pharmacists, and outside agencies.
- Performs other duties as assigned.
