About the Role

FOX Rehabilitation is seeking passionate, licensed Occupational Therapists in Goshen, NY, to provide exceptional one-on-one care to older adults in their homes or senior living communities. This role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model, offering flexibility and autonomy in managing treatment schedules.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in private residences and Senior Living Communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate.

Requirements

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, providing assistance with patient transfers, repositioning patients, standing/walking for extended periods, climbing stairs, performing therapeutic activities, and driving to patient homes.
• Must be able to wear PPE when required.

Salary/Compensations

• $70,000 - $105,000 annually for Full-Time roles in New York (pro-rated for Part-Time and FOX Flex)
