About the Role
The Case Manager, Permanent Housing provides support services to help individuals remain housed and address needs such as benefits/entitlements, activities of daily living, substance abuse and mental health services. This role involves conducting home visits, linking clients with community services, and coordinating group activities to help formerly homeless individuals transition into permanent housing.
Responsibilities
- Provide direct services to a caseload of approximately 15 clients, including home visits, assessments, and benefits assistance.
- Conduct initial and ongoing client assessments, including biannual and psychosocial assessments, and provide crisis intervention.
- Conduct psychosocial evaluations.
- Connect clients to psychiatric services.
- Connect clients to medical treatment and substance use treatment facilities.
- Connect clients to job training programs, recreational activities, community services, and food/nutrition programs.
- Assist clients in obtaining identification, public benefits, and other income sources.
- Meet weekly with clients to assess needs, encourage harm reduction, and develop service plans.
- Assist clients with documentation for recertification and housing maintenance.
- Escort clients to appointments as needed.
- Maintain accurate records in compliance with agency and governmental standards.
- Intervene in crisis situations.
- Provide assistance with self-administration of medication upon training and in accordance with agency policies.
- Perform other related duties as assigned.
Requirements
- Bachelor’s Degree or equivalent work experience.
- Two years of experience working with homeless adults and substance abuse issues preferred.
- Experience with harm reduction and motivational interviewing techniques strongly preferred.
- Valid NY, NJ, CT, or PA driver’s license with a good driving record is preferred.
- Excellent computer skills, including proficiency in Microsoft Word and Excel and the ability to learn new programs.
- Excellent writing and communication skills.
- Bilingual Spanish/English is preferred.
