About the Role
As an Oncology Nurse Coordinator, you will guide patients and their families through their healthcare journey using the nursing process to assess, develop, implement, and evaluate individualized care plans. You will serve as the primary clinical contact, providing support, education, and guidance, and coordinating multi-disciplinary and multi-site care. This role involves facilitating communication among providers, organizing education and training, and enhancing the patient experience through exceptional customer service.
Responsibilities
- Utilizes the nursing process to assess, develop, implement, and evaluate individualized plans of care.
- Acts as the central point of contact for patient and caregiver inquiries related to care plans.
- Proactively anticipates patient needs to ensure timely navigation.
- Assesses learning needs and provides education to promote adherence to established care plans.
- Enhances the patient experience by delivering exceptional customer service and promptly addressing concerns.
- Performs all responsibilities aligned with the Nurse II role, with additional emphasis on care coordination, communication, and patient advocacy.
- Interacts with patients in person, via phone, virtual visits, and the patient portal to ensure care is well coordinated.
Requirements
- License to practice as a Registered Nurse in the State of New York.
- Certification in Basic Life Support (BLS) or eligibility for certification.
- Bachelor’s degree in Nursing and the equivalent of one (1) year of full-time experience in oncology nursing, medical-surgical nursing, or critical care.
- Bachelor’s degree in Nursing and the equivalent of three (3) years of full-time medical-surgical experience in an acute care setting.
- Associate’s degree in Nursing and the equivalent of three (3) years of full-time experience in oncology nursing, medical-surgical nursing, or critical care.
- Associate’s degree in Nursing and the equivalent of five (5) years of full-time medical-surgical experience in an acute care setting.
- Incumbents must obtain and maintain applicable certification, as required by area of specialty and outlined in Departmental procedures, within thirty (30) months of appointment to this position and as a condition of continued employment.
- As a condition of continued employment, incumbents must obtain certification in Basic Life Support (BLS) within six (6) months of their appointment to this title.
- Incumbents must complete twelve (12) cancer-related contact hours per calendar year unless certified by the Oncology Nursing Certification Corporation (ONCC).
- Degrees must be conferred by an accredited school, college, or university, or by an institution recognized by Roswell Park Comprehensive Cancer Center and/or Health Research Incorporated as adhering to acceptable educational standards.
