About the Role
Change Grow Live is a charity dedicated to improving the lives of service users. This role focuses on providing specialist clinical interventions to individuals who are homeless or at risk of homelessness, aiming to improve their choice, control, and overall well-being.
Responsibilities
- Carry out physical and psycho-social assessments for the homeless population using nursing skills and knowledge.
- Provide clinical and therapeutic treatments based on evidence to improve health outcomes for the homeless population.
- Perform screening for blood-borne viruses (BBV) through dry blood spot testing.
- Work within and consolidate integrated care and treatment pathways for individuals with BBV/s (Hepatite B, Hepatitis C, HIV).
- Administer Hepatitis A and/or B vaccinations when indicated.
- Collaborate with other community teams and voluntary organizations to ensure seamless care delivery for the homeless population.
- Act as an advocate for people experiencing homelessness, supporting their choices and promoting independence.
- Contribute to the CGL Naloxone Strategy by training staff and individuals in naloxone use.
- Offer wound care assessment and basic wound dressing, organizing signposting to other services as needed.
Requirements
- First-level registered nurse with recent relevant post-qualification experience.
- Current nursing PIN.
- Demonstrable knowledge of harm reduction and health promotion interventions.
- Ability to assess and recovery plan service users and formulate written reports.
- Clinical auditing skills.
- Professional, empathetic, and non-judgmental attitude towards service users and stakeholders.
- Understanding of professional integrity in relationships with service users, peers, and other professionals.
- This post is subject to a Disclosure and Barring Service (DBS) check at an enhanced level.

- Outer London Weighting: £2,250.61
