About the Role

The Youth Health Centre Coordinator (YHCC) reports to the Manager Healthy Communities and is an active member of the Healthy Communities Public Health Team. The YHCC provides leadership, informal assessment, service navigation, youth engagement, and support for youth within an assigned school facility and its community. This role champions youth health and health equity, facilitating youth engagement as part of a health-promoting schools approach. The position collaborates with various school and community partners to ensure youth access to support, programs, and services.

Responsibilities

Provides leadership, informal assessment, service navigation, youth engagement, and support for youth associated with the assigned school facility and its immediate community.
Champions youth health and health equity.
Facilitates youth engagement as a component of health-promoting schools.
Works with youth and fosters cooperation and collaboration among school administrators, teachers, SchoolsPlus, guidance, student/youth support workers, primary health care, mental health and addictions, and other community partners.
Models and provides safe, equitable, and inclusive youth support.
Understands the diverse experiences of the school and the surrounding community.

Requirements

Baccalaureate degree in a relevant health care discipline (e.g., registered nurse, occupational therapist, social work, nutritionist).
Minimum of three years’ experience in a relevant field, directly working with youth.
Registered/licensed with your regulated health care discipline, college or association (registered nurse, occupational therapist, social work, nutritionist).
Must be bilingual in French and English, both written and verbal.
Must have a valid driver’s license and access to a reliable vehicle.
Ability to work independently and as a member of a multidisciplinary team, both within the organization and in the community.
Ability to critically assess situations and navigate complex scenarios using sound decision-making and a non-judgmental, trauma-informed approach in supporting youth.
Demonstrated ability and experience in creating opportunities for meaningful youth engagement and fostering youth leadership.
Demonstrated understanding of the social determinants of health, health equity, and their impact on health outcomes for youth, families, and communities.
Experience in the application of public health principles and practices, partnership development, and community development.
Experience relevant to children and youth, health promotion strategies in school and community settings is preferred.
