About the Role

Join Yee Hong as a Social Worker at the new Ng Siu Chan Centre, a long-term care home focused on Relationship-Centred Care and Meaningful Living. Help shape the culture and resident experience from day one in an innovative environment that promotes purpose, dignity, independence, and choice.

Responsibilities

  • Utilize professional case management practice for intake assessment, service coordination, referrals, case conferencing, and monitoring.
  • Provide supportive counseling to clients and their family caregivers individually and in groups.
  • Coordinate with interdisciplinary team members to implement and monitor resident care plans related to social work support.
  • Offer instrumental help to clients to resolve daily living functional problems.
  • Provide crisis intervention and short-term assistance while maintaining a caseload.
  • Develop and implement social work service policies in consultation with the Executive Director.
  • Participate in and contribute to interdisciplinary team conferences within the LTC Home.
  • Collaborate with the Executive Director to provide social work services to LTC Home residents.
  • Actively participate and contribute to overall LTC leadership team meetings and functions.
  • Stay current with developments and best practices in the field.
  • Perform other duties as assigned.

Requirements

  • Bachelor degree in Social Work or equivalent.
  • Registered Social Worker designation in Ontario.
  • Minimum 2 years of experience in counseling or group work, preferably in a long-term care setting.
  • Proficiency in computer applications.
  • Ability to work independently, be self-directed in problem-solving, and exercise good judgment, tact, and diplomacy.
  • Empathetic attitude and a desire to contribute to the well-being of the elderly.
  • Excellent communication and interpersonal skills.
  • Satisfactory Police Vulnerable Sector Screen result.
  • Ability to converse in Chinese.
  • Must provide a recent satisfactory vulnerable sector police reference check and medical clearance certificate.
