About the Role

Bristol Hospice is seeking an experienced Hospice Chaplain to join their team. This pivotal role involves meeting the spiritual needs of patients and families through assessments, care plans, and collaboration with the Interdisciplinary Team. Bristol Hospice is a nationwide leader focused on a family-centered approach to hospice care, emphasizing compassion, respect, and dignity.

Responsibilities

• Assure spiritual assessment of patients and families/caregivers and render appropriate services in a timely manner.
• Facilitate the development of the individualized plan of care by participating in comprehensive assessments to meet identified spiritual needs.
• Provide direct spiritual care to patients and families/caregivers.
• Serve as liaison and support to community chaplains and spiritual counselors.
• Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policy review.
• Document direct services and ongoing communication with community chaplains and spiritual counselors.
• Attend patient care conferences and interdisciplinary group meetings.
• Provide consultation, education, and support to the interdisciplinary group on spiritual care.
• Recruit community chaplains, spiritual counselors, and spiritual care volunteers by developing community contacts and offering education about hospice care.
• Provide for funeral or memorial services for patients as requested.
• Plan periodic memorial services for personnel, volunteers, and community clergy/spiritual counselors.
• Assist in the supervision of spiritual care volunteers when assigned to patients/families/caregivers.
• Actively participate in quality assessment performance improvement teams and activities.
• Accept and perform other related duties and responsibilities as required.
• Perform other duties as assigned.

Requirements

• Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree preferred.
• Must have experience working with death and dying, individuals/family/caregiver.
• Hospice experience preferred.
• Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.
• Must be willing to travel to hospice locations as assigned.
• Must be flexible with working hours.
