About the Role

Constellation Health is seeking a dynamic Licensed Practical Nurse (LPN) to join our team in Nassau County. Our LPNs provide care for clients and patients in their home or community setting, assisting in planning care under the direction of an RN. Responsibilities include following the plan of care, providing treatments, and collaborating with the team to achieve positive patient outcomes, while supporting the company's philosophy, goals, and culture.

Responsibilities

Provides nursing care as defined in State Nurse Practice Act, including specialized duties related to end-of-life care.
Implements current nursing practice following a comprehensive assessment and the plan of care.
Maintains accurate and timely documentation of patient services to reflect the plan of care.
Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
Recognizes and interprets symptoms and institutes remedial measures within the scope and limits of a Licensed Practical Nurse.
Observes, records, and reports any changes in patient's condition to immediate supervisor.
Participates in the coordination of hospice and other home care services, reporting identified needs to the interdisciplinary group (e.g., RN, hospice aide, MSW, OT, PT, ST, Registered Dietician, Spiritual Counselor, Clinical Supervisor).
Participates in Hospice IDT meetings.
Implements care to achieve outcomes of comfortable and safe dying and effective grieving for the patient and family.
Uses and prepares equipment and supplies effectively, adhering to aseptic technique.
Provides appropriate pain/symptom management.
Evaluates patient's response to treatments/medications.
Participates in personal, professional growth, and development.
Participates in the organization's quality assessment performance improvement team activities.
Performs other duties as assigned by the registered nurse or clinical supervisor.
Some weekend days are required, and summer and winter holiday commitments as needed.

Requirements

Graduate of an accredited practical nurse program.
Licensed LPN in the state of employment.
At least 1-2 years of LPN experience is a MUST; Home care preferred.
Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care is a plus.
Must have reliable transportation and be willing to commute to provide in-home patient care.
Alternating weekend days are required, and summer and winter holiday commitments as needed.
Basic knowledge of iOS operating system.
