About the Role

Navigators play a critical role in CHASI's Prevention Department by addressing barriers clients face in accessing services. They provide case management and referrals to comprehensive HIV prevention services for priority populations.

Responsibilities

  • Conduct intakes, mental health, and SUD screenings and other assessments as needed.
  • Educate and counsel persons on prevention strategies and materials.
  • Develop, coordinate, and integrate service plans in collaboration with clients.
  • Assist clients with appointment scheduling, provider follow-ups, logistics, reminders, transportation, and accompanying clients to appointments.
  • Perform and maintain effective case management for an assigned caseload from intake to discharge.
  • Track and monitor client progress and maintain detailed, accurate, and timely notes.
  • Provide appropriate internal and external referrals and collaborate with partner providers.
  • Provide linkage, coordination, referrals, and follow-up with appropriate ongoing providers, including PrEP, PEP, iART/HIV care, and STI treatment.
  • Ensure client connection and compliance with appointments and action plans, and address barriers.
  • Work on weekends as needed to support outreach events and community engagement activities.
  • Escort clients to appointments and conduct home visits, as needed.
  • Collect and submit accurate data on persons contacted and engaged in services.
  • Maintain updated and accurate records on services and activities provided.
  • Participate in documentation quality assurance and reconciliation tasks.
  • Keep up to date on the program client list and deliverable tracker.
  • Participate in training, professional development activities, and meetings.
  • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values.
  • Perform other duties as assigned and participate in special projects.
  • Occasionally work nights and weekends.

Requirements

  • At least 2 years of experience providing case management or health education with high-risk populations.
  • High School diploma or equivalent required; some college preferred.
  • Bilingual (English/Spanish) preferred.
  • Basic knowledge regarding HIV, HCV, STI, PEP/PrEP and Harm Reduction methods.
  • Preferred experience in PrEP AP, Gilead, and applying for copay cards.
  • Excellent MS Office skills (Outlook, Word, Excel, PowerPoint).
  • Ability to work independently and as a team player with diverse teams.
  • Ability to contribute to a positive, productive, open, and supportive environment.
  • Ability to interact with all levels of external and internal partners with professionalism and tact.
  • Strong verbal and written communication skills.
  • High attention to detail and accuracy.
  • Highly organized and able to multitask.
