About the Role

Open Door is seeking a Care Manager, Youth Treatment & Recovery to provide ongoing care coordination services to patients enrolled in the Family Tree Program. This program enhances treatment and recovery services for vulnerable adolescents and transitional aged youth with substance use disorders and/or co-occurring mental health disorders. The Care Manager will work within a patient-centered medical home model, participating in a team-based approach to improve patient health outcomes and empower patients to manage their health.

Responsibilities

Outreach individuals and engage them in the Family Tree Program.
Conduct assessments to identify enrollees’ individualized needs related to psychosocial wellbeing, physical health, and social determinants of health.
Collaborate with enrollees to develop a plan of care based on assessment findings.
Engage enrollees in goal setting and utilize Motivational Interviewing to assist in goal achievement.
Coordinate and arrange for the provision of medical, behavioral health, and other relevant services.
Accompany enrollees to appointments as needed; model and coach on medical/behavioral visit navigation.
Conduct home visits as needed.
Support enrollees in adherence to treatment recommendations.
Provide psychoeducation on physical and behavioral health conditions and treatments.
Identify community-based resources and manage referrals, access, engagement, follow-up, and coordination of services.
Promote evidence-based wellness and prevention by providing health education on substance use, mental health, and chronic medical conditions.
Link enrollees to services/resources based on individual needs and preferences.
Discuss advance directives and health care proxies with enrollees and their families or caregivers.
Retain enrollees in care for a period of at least 6 months.
Travel as required to community-based meetings, provider sites, and meetings with enrollees.
Represent Open Door at community health fairs and outreach activities.
Complete all required documentation and data entry for the Family Tree Program.
Inform supervisor promptly of any event that may affect quality and standards of patient care or the success of the Family Tree Program.
Function in a culturally sensitive manner within Open Door’s philosophy and mission.
Actively promote planned care concepts within Open Door.
Attend staff and other organizational meetings to ascertain Open Door’s short- and long-range plans, programs, and goals.
Participate in staff development programs, educational activities, seminars, and training opportunities.
Adhere to organizational and federal requirements regarding data and confidentiality, electronic transmission protocols, and security.
Maintain patient confidentiality at all times per Open Door policies and procedures.
Adhere to National Patient Safety standards.
Prepare and maintain documentation in compliance with Joint Commission standards and applicable laws.
Meet annual mandatory training requirements.

Requirements

Must have valid NYS driving license and daily access to a car.

Experience Level

At least three years of experience in substance use treatment, mental health, healthcare and/or social services settings
Health education experience is preferred
Experience with electronic medical records is preferred
Knowledge of/experience in case management and care coordination is preferred
Experience working with young people aged 12-25

Education Level

Master’s Degree preferred
Bachelor’s Degree with solid work experience considered
Degree in Social Work or social sciences preferred
Other degrees with relevant experience considered
Certification or coursework in addiction, substance use disorder treatment and management preferred (e.g. CASAC or CASAC-T/CASAC-P)
