About the Role

Options for Community Living, Inc. is seeking a Case Manager to provide supportive case management services to individuals with mental illness in permanent housing. The role involves monitoring housing sites, evaluating service plans, and connecting residents with community resources.

Responsibilities

  • Provide supportive case management services to individuals with mental illness in permanent housing.
  • Monitor maintenance of sites and provide on-site visits to scattered housing locations.
  • Plan and evaluate service plans and monitor objectives.
  • Write progress notes at least bi-weekly.
  • Review case management plans and prepare periodic reassessments.
  • Perform site visits 1-3 times weekly dependent on client need and desire.
  • Provide information regarding community resources and orient residents to the area.
  • Assist in securing vocational assessment services, job training, and education programs.
  • Provide advocacy services to gain appropriate entitlements and services.
  • Intervene in situations requiring immediate attention to ensure resident safety.
  • Provide mediation, counseling, behavioral intervention, and crisis management services.
  • Consult and coordinate with clients’ families and providers as needed for coordinated service delivery.
  • Notify program supervisor/manager of any medical or psychiatric emergency.
  • Oversee medication maintenance and assist residents in the development of self-medication skills as needed.

Requirements

  • High School Diploma or the equivalent and 2 years of qualifying experience, or a Bachelor's degree and 1 year of qualifying experience.
  • Excellent time management skills.
  • Valid driver’s license, safe driving record, valid auto insurance, and access to a vehicle is required.
  • Qualifying experience: verifiable full or part-time case management or case work with persons with a history of mental illness, homelessness, or chemical dependence.
