About the Role
The Case Aide performs various supportive activities for casework services in support of Preventive Services programs. Duties include interviewing clients, collecting information, assisting clients with transportation and home visits, and participating in case conferences.
Responsibilities
- Work closely with Case Planners to provide collateral services to individuals and families.
- Attend JHV with Case Planners and CPS at case initiation.
- Assist and model for parents and children methods to achieve service plan goals.
- Participate in supportive services such as recreation and educational programs.
- Carry out ongoing quarterly risk and safety assessments and document them in Connections.
- Bring all risk/safety concerns to the immediate attention of their supervisor.
- Request all school and medical reports and document them in Connections.
- Assist families in accessing in-house and community-based services.
- Complete referral applications and gather required support documentation.
- Enter progress notes in CONNX within 5 days of occurrence.
- Submit hard copies of progress notes in draft form weekly to supervisor.
- Insert copies of all printed progress notes in case file.
- Enter all casework contacts in PROMIS weekly.
- Make required home visits and collateral contacts to meet ACS minimum contact mandates.
- Ensure the safety of children and families during visits.
- Assist with office clerical duties, including petty cash management and supply ordering.
- Serve as receptionist, answer telephones, and take messages.
- Assess cases for health home/GABI referral and initiate the process.
- Participate in mandatory supervision, trainings, and staff meetings.
- Submit weekly calendar of scheduled visits to supervisor.
- Check files for necessary updated documentation and assessments.
- Perform other duties as assigned.
Requirements
- Bachelor’s degree strongly preferred plus a minimum of two years of prior experience as an Office Assistant.
- High School Degree or GED equivalent considered with prior experience in child welfare environment.
- Demonstrate strong organizational and computer skills including proficiency with Microsoft and Outlook.
- Driver’s license a plus.
- Bi-lingual (English and Spanish) a plus.
- Minimum typing speed of 60 WPM.
- Ability to carry out assignments with attention to accurateness, thoroughness and timeliness and with minimal supervision.
- Must be open to traveling across the NYC area.
- Demonstrated ability to relate positively to people from diverse backgrounds and lifestyles.
- Must be LGBTQ affirming.
- Excellent interpersonal & customer service skills.
- Physical requirements include the ability to lift and reach for light objects.
- Close visual acuity to prepare and analyze data, text, and figures.
- Ability to type utilizing a computer keyboard.
- Ability to travel to outside locations.
- Ability to work at a computer for extended periods of time as a primary job function.
- Authorized to work in the U.S.
- Ability to work in-person in New York City (NY) for at least 4 days or more per week.
- Ability to travel to other Rising Ground sites (if required).
- Ability to travel within the community for visits.
