About the Role

The Case Manager is responsible for developing, implementing, and monitoring Life Plans for individuals with developmental disabilities in a residential setting. This role ensures services align with regulatory requirements, agency standards, and individual goals, while promoting dignity, independence, and quality of life.

Responsibilities

  • Provides social services to maximize social functioning and coping capacity.
  • Develops, monitors, and updates Individualized Service Plans.
  • Participates in and chairs interdisciplinary team meetings.
  • Collaborates with residential, clinical staff, and service providers for coordinated support.
  • Identifies individual aptitudes and develops person-centered goals and activities.
  • Documents functional assessments and reviews plans of care.
  • Provides supervision, training, and guidance to Residence Program Specialists.
  • Maintains regular contact with families, guardians, and community stakeholders.
  • Advocates for individuals' rights and ensures timely application for benefits.
  • Coordinates admissions, including referral materials and family contacts.
  • Serves as liaison to day programs, clinics, and external partners.
  • Provides support in activities of daily living and community outings.
  • Ensures adaptive equipment is available, maintained, and used appropriately.
  • Maintains accurate and timely documentation, including case notes and progress updates.
  • Assists with discharge planning and follow-up for transitions to community-based services.
  • Handles sensitive and confidential information with discretion.
  • Stays updated on field changes and applies new knowledge to practice.

  • Ability to read, write, speak, and understand English.
  • Strong interpersonal and communication skills.

  • Ability to handle multiple priorities.
  • Proficiency in Microsoft Office and electronic record systems.
  • Ability to maintain confidentiality and exercise discretion.
