About the Role

The Habilitation Support Professional (HSP) supports individuals with Intellectual and Developmental Disabilities in Home and Community Based Settings (HCBS) and Day Habilitation programs. The HSP follows Individualized Service Plans (ISP) and/or Day Habilitation Plans to promote independence and person-centered decision-making.

Responsibilities

  • Ensure client safety in home or day habilitation settings.
  • Promote client independence and meet client needs and choices in a person-centered manner.
  • Attend to clients' personal hygiene needs.
  • Collaborate with clients, guardians/advocates, and supervisors to identify valued outcomes for habilitation plans.
  • Implement valued outcomes daily using agency documentation procedures.
  • Develop and implement activities with clients and primary caregivers.
  • Accompany clients on trips and outings, supervising activities.
  • Participate in interdisciplinary team meetings to review progress and plan future outcomes.
  • Share and document client experiences and observations with the treatment team.
  • Contact supervisors for any medical or behavioral occurrences outside the client's baseline.
  • Understand and follow all safeguards listed in individual treatment plans.
  • Dispense medications in Day Habilitation settings if needed during work hours.
  • Complete OPWDD documentation requirements, including monthly checklists and summaries.
  • Adhere to agency guidelines for timekeeping and complete mandatory agency trainings.
  • Complete ongoing OPWDD and other required trainings.

Requirements

  • High school diploma or a minimum of 1 year of experience working with the IDD population.
  • Must be at least 18 years of age.
  • AM AMAP certification required for the Day Habilitation setting during the probationary period.
  • Basic computer skills required, including agency software programs and Electronic Health Record.
  • Ability to review treatment plans and document daily logs.
  • Dexterity to pick up small items for client activities.
  • Ability to lift clients using a mechanical device, position clients, push clients in wheelchairs, and physically transfer clients where prescribed.
