About the Role

Open Door Family Medical Center is seeking a Patient Advocate to join their medical care team. The Patient Advocate will aim to improve patient health outcomes and assist patients in managing their own healthcare, becoming independent and informed consumers of health care services.

Responsibilities

• Provide basic education on chronic diseases to patients and caregivers.
• Reinforce medical provider’s guidance on basic self-management activities for chronic diseases.
• Develop and support adherence to care plans for patients with chronic diseases.
• Assist patients in establishing self-management goals and creating action plans.
• Make timely referrals for identified needs related to social determinants of health.
• Assist patients with completing referral paperwork.
• Serve as a healthcare system navigator to ensure patient access to services.
• Educate patients about low-cost pharmaceutical drug programs and facilitate enrollment.
• Assess patients’ needs related to social determinants of health using a standardized screening tool.
• Serve as a liaison between primary care providers and patients for specialty medical services.
• Assist patients with preparation for medical specialist appointments.
• Collaborate with the medical care team to acquire records from outside medical specialists.
• Assist patients with navigating the cancer screening process.
• Assist patients to prepare for cancer screening appointments.
• Complete enrollment for the New York State Cancer Services Program (CSP).
• Send correspondence to patients for cancer screening test notifications.
• Assist patients with cancer diagnoses in navigating the treatment process.
• Refer patients to support resources as needed.
• Collaborate with the medical care team to acquire records from cancer screening and treatment.
• Document all patient care activities in the Electronic Medical Record clearly and accurately.
• Conduct other data tracking and reporting activities as assigned.
• Conduct screenings and assessments.

Requirements

• Bilingual English/Spanish with written and verbal fluency in both languages.
• Valid NYS driver’s license and daily access to a car.
• High School Degree required.
• Customer service experience required.
• Experience with community and health organizations preferred.
• Passionate about providing high-quality care to low-income, underserved individuals.
• Able to function effectively as a member of an interdisciplinary team.
• Culturally competent/sensitive and committed to inclusive, multicultural programming.
• Proficient computer use, including Microsoft Office applications.
• Able to understand and follow detailed instructions.
• Highly organized and self-directed, able to multi-task.
• Flexible and exhibits the ability to adapt as situations evolve.
• Comfortable working in a fast-paced, high accountability environment.
• Effective in problem-solving and exhibits excellent judgment.
• Ability to prioritize.
