About the Role
Liberty Home Care is seeking a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor. This Part Time opportunity offers great benefits and a consistent M-F 9a-2p - 25 hour per week schedule.
Responsibilities
- Provide assessment of newly referred patients and assess health needs in home care setting
- Work with patient to create a patient centered plan of care
- Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family
- Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services
- Provide supervision and training of PCA/HHA staff
Requirements
- NYS Licensed Registered Professional Nurse
- One year Nursing experience required
- Home care experience a plus
