About the Role

As a Home Visitor for Clinton and Franklin Counties, your community and child-focused approach to care will help North Country children and their families partner with the community to create happy, health, safe and school ready children.

Responsibilities

  • Develop and perform community outreach and assessments for expectant and new parents.
  • Connect families to resources to support their baby's needs in cognitive, physical, social, and emotional development, health, and safety.
  • Assist with bonding and attachment for families.
  • Conduct quality assurance reviews of participant files to ensure adherence to HFA and HFNY standards.

Requirements

  • High school diploma and seven years' related experience required.
  • Bachelor's degree plus three years' related experience preferred.
  • College coursework counts towards related experience.
  • Demonstrable knowledge of local/community resources.
  • Ability to work a flexible schedule to meet family needs.
  • Strong engagement, communication, time management, and organizational skills required.
  • Valid NYS driver's license and reliable transportation required for travel.
