About the Role
We are seeking a motivated Social Worker with excellent leadership and communication skills to assist in planning, developing, organizing, implementing, and evaluating social service programs. You will also participate in discharge planning, resident assessments, and coordinate social service activities.
Responsibilities
- Assist in planning, developing, organizing, implementing, and evaluating the social service programs.
- Assist in developing and implementing policies and procedures for identifying medically related social and emotional needs of residents.
- Participate in discharge planning, development, and implementation of social care plans and resident assessments.
- Interview residents and families to obtain family history.
- Perform administrative requirements, including completing necessary forms and reports.
- Involve residents and families in planning social service programs when possible.
- Assist in arranging transportation to other facilities when necessary.
- Refer residents and families to appropriate social service agencies when the facility does not provide the needed services.
- Coordinate social service activities with other departments.
- Interpret the social, psychological, and emotional needs of residents and families to the medical staff, attending physician, and other resident care team members.
- Review department complaints and grievances from personnel and report actions taken to the Administrator.
- Follow facility's established procedures.
Requirements
- Skilled Nursing Facility Experience: (Required)
- Social Work License (Preferred)
