Responsibilities
- Provide Family Centered Treatment (FCT) Clinical Services as per the service definition and model fidelity requirements.
- Complete FCT Checkoffs and other internal training within designated deadlines.
- Deliver FCT clinical services to children, adolescents, and their families, primarily in their living environments, with a family focus.
- Provide 24/7 on-call coverage to a caseload of families and children.
- Complete all required Clinical Records and FCT documents with the child and family and ensure timely entry/upload into the Electronic Health Record.
- Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes.
- Work collaboratively on behalf of the child and family with identified Treatment Team members.
- Provide psychoeducation as indicated in the Service Plan.
- Conduct interventions with the family aligned with the FCT model, facilitating movement through the four phases of treatment and completing fidelity documents.
- Assist families and children to establish weekly goals and identify steps to reach them.
- Develop and coordinate discharge plan based on treatment, indicating aftercare needs and plans.
- Access community resources beneficial to client aftercare needs.
- Coordinate and oversee initial and ongoing assessment activities.
- Convene the Treatment Team for Service Plan.
- Consult with identified medical and non-medical providers; engage community and natural supports and include their input in the person-centered planning process.
- Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
- Use NPI number to bill for services performed by those being supervised.
- Ensure credentials remain current, comply with all training requirements, and adequately supervise staff.
Requirements
- Master’s Degree in Counseling, Social Work, or related field is required.
- Two years’ experience working with youth or families.
- Current Family Centered Treatment Certification is preferred.
- Must hold one of the following current professional licenses in the state of Ohio: LSW, LISW, LPC, LMFT, or LPCC.
- Employment is contingent upon meeting the requirements of a comprehensive background investigation.
