Responsibilities
- Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
- Complete FCT Checkoffs and other internal trainings within designated deadlines.
- Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
- Provide 24/7 on-call coverage to a caseload of families and children, requiring a flexible work schedule.
- Complete all required Clinical Records and FCT documents with the child and family and ensure timely entry/upload into the Electronic Health Record.
- Develop and maintain Service Plans in coordination with the Treatment Team Process.
- Work collaboratively with Treatment Team members for effective communication and coordinated service provision.
- Conduct ongoing revisions of Service Plans and ensure their implementation.
- Provide psychoeducation as indicated in the Service Plan.
- Conduct interventions aligned with the FCT model, facilitating movement through treatment phases and completing fidelity documents.
- Assist families and children in establishing weekly goals and identifying steps to reach them.
- Make referrals to services and resources on behalf of families and children.
- Develop and coordinate discharge plans based on treatment, indicating aftercare needs and plans.
- Access community resources beneficial to client aftercare needs.
- Coordinate and oversee initial and ongoing assessment activities.
- Convene the Treatment Team for Service Plan development.
- Consult with identified medical and non-medical providers, engaging community and natural supports in the person-centered planning process.
- Ensure linkage to the most clinically appropriate and effective services, including arranging for evaluations.
- Provide and coordinate behavioral health services and other interventions with other professionals and Treatment Team members.
- Monitor and document consumer progress.
- Participate in Quality Assurance/Quality Improvement activities.
Requirements
- Minimum of a Bachelor's Degree in human services or related field is required.
- Two years’ experience working with youth or families.
- Current Family Centered Treatment Certification is preferred.
- Ohio specific requirements: LSW or eligibility for provisional licensure preferred.
- Employment is contingent upon meeting the requirements of a comprehensive background investigation.
